What is likely happening is that the extension is fine but some shared library that is getting loaded at boot is corrupt. This is a classic extension conflict problem.

Try leaving the extension enabled and disable all the Shared Libraries. Create a folder in the Extensions folder called Shared Library (disabled) or such. Then reboot and see if you still have the problem. If it goes away then start trying the shared libraries either one by one or using a successive approximation - try half, if it boots okay try half the remaining, if it doesn't boot then boot half of the test lot back in the disabled folder.
