Despite the fact that you confirmed drive termination, these symptoms are
very typical of an improperly terminated SCSI drive. It may not be simply
the fact that the drive is or isn't terminated, but the termination power
setting.

To see if this might be the problem, I would put the drive in an external
box, and use an external terminator, removing the termination jumper on the
drive itself. Then start from a system CD and see if I couldn't reformat the
drive.
