Dear Peter,
I've had exactly the same symptoms, and it drove me batty until
someone else on this list helped me out. The card has worked loose.
Open the case and push it firmly back into the slot, till it clunks.
Then do a Cuda/Pram zap. Fixed it first time. There's even an Apple
Tech note on it.
