I had the same problem in my beige G3 minitower. I ended up replacing the Belkin USB card with a relatively cheap Firewire/USB 2.0 combo card I bought on eBay (around $40-50). Everything seems to be working just fine now, and my USB 2.0 card reader downloads digital photos MUCH faster.

If I remove the usb card, video is fine in both os's. If I leave it in, I boot to a black screen. The mouse appears to initialize early on the few occasions I make it to video, and then everything freezes. So far, I've tried swapping slots, zapping the pram (losing Jaguar twice), replacing the pram battery, and resetting the cuda switch (also another excellent way to trash OS X). Has anyone heard of conflicts between the Radeon and other pci cards, or do I just have an unlucky combination with the scsi card thrown in?
