Have you tried trashing the Display prefs from the prefs folder, also the one in the folder Display prefs in the prefs folder? Plus the one created by the 3dfx drivers, I think it was called "3dfx prefs". If not you could try that, then reboot and zap the PRAM. And did you disable the ATI extensions plus the OpenGLRendererATI extension? And I recommend the Voodoo 1.1.2 drivers or the 1.1.3b. Don't use the Voodoo 3 drivers such as b1, b2 until b13. And are you using the DVI or VGA connecter? If one doesn't work see if the other does. BTW you can't use both connectors at the same time. Also if all else fails try booting with the monitor disconnected or connected to the onboard video, and when it's booted connect the monitor to the V5 to see if you get a signal.
