spammers traditionally diddle with the message headers in all kinds of ways to disguise themselves and confuse other parts of the system, it's possible to send email to someone and have it show a different destination address. if you are curios, select the option to view all headers or "source" and read it, it's almost amusing sometimes. additionally, one of the reasons hotmail exist is to send their users spam, the same goes for yahoo although i believe that with yahoo you can opt out when you sign up if you read all the options, or you may have to send them an email/mail to opt out. microsoft complains about spam publicly, but encourages it on hotmail accounts and sells the privilege in addition to the many spammers who have just harvested email addresses.
the best tool for reporting spam or just finding out where it came from is spamcop.net, you can report spam for free and their program will process the email to see who to complain to. they also have a paid service for mailboxes that are filtered for spam, a friend of mine has that service and hasn't had any spam in over a year! they've also been sued by spammers and naturally they've won so far (which shows you how much the spammers hate them and how well organized some of them are, the direct marketing association says that spam is a consumer service for instance, yes, it's very, very helpful to have so many people worried about your penis...).
