Apple shipped their own adapter with every B&W, no dip switches. Track one of these down or find any NEC or similar adapter.
Row A Switches 2 & 4 On Row B Switches 7 On = 832x624
Row A Switches 2 & 3 On Row B Switches 7 On = 1024x768
Make sure all of the others are reset by pushing them to the ON position and then back to the OFF position.
Also zapping the PRAM and deleting display pref's doesn't hurt.

My B&W 17" Studio Display connects directly to either my G3 B&W 350 or my G4
Sawtooth 450, both computers still have stock video cards.
No adapter is needed.
Can't you just connect the monitor your G3 B&W directly without the adapter?
BTW both machines run OS 10.3.7 and OS 9.2.2 --glen (digest mode)
