The B&W G3 Rev1 is known to have data corruption with certain drives. Especially when using a slaved HD since the Rev1 does not work with slave HDs at all. Buying a PCI IDE controller would solve this problem. Of course the HD could be bad anyway, you said you formatted the HD several times but have you also done a low level format? For the heck of it I would go out and get a new IDE drive, since they're cheap nowadays, and get a PCI IDE controller a long with it. OWC is a good reseller (www.macsales.com. Not affiliated BTW).
