Using your photo as legend:

The bottom brown slot is for an internal modem (as per the
www.xlr8yourmac.com/G3-ZONE/modem/ link that Doug provided).

The other two, I believe, are used for some sort of interface when the card
is used in an All-In-One G3 (the "molar"). The odd black one with the "arms"
is, iirc, supposed to have a break-out cable that at other end provides an
external video option (classroom purposes?) for the user that would
otherwise be the only one able to see the screen. I have never seen such a
cable for the G3, though I did for the Performa and Pmac 5x00 series (same
concept, I'm presuming) - maybe somebody has a photo? Or maybe this was a
future development situation that never materialized, thanks to the
personality card's quick demise.
