The other two, I believe, are used for some sort of interface when the card is used in an All-In-One G3 (the "molar"). The odd black one with the "arms" is, iirc, supposed to have a break-out cable that at other end provides an external video option (classroom purposes?) for the user that would otherwise be the only one able to see the screen. I have never seen such a cable for the G3, though I did for the Performa and Pmac 5x00 series (same concept, I'm presuming) - maybe somebody has a photo?
