Lots. I've been using it for 8 or 9 years now, currently running something like 100 small domains through it.
Get the 1.8b9d14 version. It's officially a beta, but it's been rock solid stable for years. Stalker isn't going to be updating it anymore.
It can be run in classic mode (it's only available in OS9) but in my experience it's a little dodgy there. Stick to OS9.
Obviously, it's getting a little long in the tooth. There are some recent changes which it doesn't include, like SPF support and IMAP.
For those reasons, I'm preparing to move over to another mail server in the near future. But it your needs are modest and you have an old OS9 machine kicking around you can do a lot with it.
I also recommend using ASSP (http://assp.sourceforge.net) in conjunction with SIMS. Using blacklists with SIMS works fairly well for spam catching, but ASSP is far better, even without using its blacklist ability
