Your HD driver is corrupted. You can boot an OS 9.x Install CD using
the "C" key and then go to Utilities folder>Drive Setup. Highlight
the HD and use the function "Update Driver". If you're in OS X it may
say "the driver you are using is newer than the one you are
installing, are you sure you want to update?" which you should answer
"yes". You might as well run Disk FirstAid also from the CD since
you've taken the time to boot it.
If you can't boot the CD with the "C" key at boot, you'll need to
reset the PRAM & NVRAM.
To reset the PRAM hold the keys Cmd-Opt-P-R until you hear the chime
again (you might let it chime a few times if you want).
You can then continue on the reset the NVRAM by listening for the
chime and immediately changing the fingers on P & R to O & F so that
you're now pressing Cmd-Opt-O-F.
This will boot into Open Firmare, and you should see a white screen
with a ">" prompt.
If you're using a PCI video card instead of the onboard video you may
not see anything, the monitor may remain black, but you are probably
still booted into Open Firmware and it's just not showing on the
monitor.
Either way, type carefully:
set-defaults<Return>
reset-nvram<Return>
reset-all<Return>
<Return> means hit the Return key. It should reply "ok" to the first
two commands and then restart after the third with a chime which is
useful if your monitor is black and you need confirmation that you've
successfully reset the NVRAM.
