Hi Justin, 
Thank you for the response. I've reseated the card three times, so I doubt
that #2 is the concern. #1 is still a possibility (one person reached me
off-list saying that he had that card in several different G4s with
success).
The card itself, to answer your last query, has twp ports, one VGA and one
ADC. He most overt declaration of a model on it is a small stamp saying
"NVidia Model no. P40" - there is no Apple Part no. on it, nor anything that
specifically says "Mac" on it (except the ADC port).
