Hi Clement, Yes, there are random items and random monsters in the game. That feature has been around since mid 2008 or so. I really suggest you try beta 12 or wait for beta 13 and play the demo. It sounds to me like you haven't touched the game in at least two years and the game has totally changed from what you remember. Most of your questions would be answered by actually trying the current demo.
