[-f] should say that node needs to be marked as offline first.

The issue is with instances on crashed node.
On the face of it one expects gnt-node failover and gnt-instance failover
to be able to "move" instances off a dead node.
Especially since the desciption of the --ignore-consistency flag makes it
sound like it can be used to move instances from dead nodes.
It is not documented anywhere in the gnt-node failover and gnt-instance
failover commands that to failover a instance from a crashed node you need
to manually set the node's role to offline first.
