It should be noted that there are really three different kinds of
entries there:

 1) Clarifications: the meaning can be teased apart from the spec, but
    is non-obvious. Example: "Issue SOME".

 2) Further specifications: items that do not conflict with the spec as
    it is now. Example: "Issue MAKE-INSTANCES-OBSOLETE-SYMBOL"

 3) Minor revisions: items that do conflict with the spec, but are
    low-impact in terms of migration cost. Example: "Issue
    CONSTANTLY-MULTIPLE-VALUES".
