Tidied up chapter 11, removed warnings that only apply to GATE version 3 or 4,
updated the example code to use @CreoleResource annotations, mentioned the
javadoc target in "adding plugins to the nightly build", suggested texlive from
MacPorts rather than tetex from fink to build the user guide on Mac OS X.
