I suspect the bug occurs because libgfortran uses the weak symbol version of
quadmath_snprintf(). Unfortunately, I don't have the resources to verify this
in the near future.

If you could recompile gcc/gfortran with the attached patch applied, and check
my test case in https://gcc.gnu.org/bugzilla/show_bug.cgi?id=50201#c9 I would
be very grateful.
