[Fortran-caf] Merge of the trunk into the branch

Hi all,

I have now merged the trunk into the branch. The main change is the new 
co_min/max/sum feature but also a patch which has reduced the 
differences between the trunk and the branch. - And of course, all the 
other trunk changes such as the wide-int patch.
