Open your un-modified to-220 footprint, either in PCB or in a text-editor, and add the lines of your heatsink. You will then save it as a new footprint, like TO-220__20x32mm_heatsink.fp. When you gattrib your design before building the PCB you can then change the TO-220.fp to the custom one you created. The same rules can apply for vertical or flat heatsink mounting, and since there are so many different to-220 heatsinks in the world you'll end up making footprints for the ones you can get easily or prefer using.
