What's happening in my area is the insurance companies create their "networks" 
which doctors agree to certain price caps, or they don't.  If you go to an 
"out of network" doctor/dentist/etc... not only do you pay higher prices 
(because the caps don't save you) but the insurance company will often cover 
less percentage-wise as well.

And I'm paying through the nose for insurance compared to 4 years ago.

Yes, but how high a tax do they pay for it?  In Germany a few years ago they 
were paying 50% of their income in taxes.
