You would need to read the RefSeq documentation 
(http://www.ncbi.nlm.nih.gov/RefSeq/), or try contacting the NCBI 
helpdesk, to find out more about the changes RefSeq typically makes to 
their accessions.

I can tell you how to find the revision history of any particular 
identifier, though:  on the Nucleotide page at NCBI for a particular 
identifier, in the top-left corner of the page, select:

Display Settings-> Revision history-> Apply.

You will see a page like this one:
http://www.ncbi.nlm.nih.gov/nuccore/NM_214677.1?report=girevhist
which contains the entire history of changes made to the identifier.
