That's true, it can.  But *usually* it doesn't.

No, it can't. You have to tell it to so explicitely. Otherwise it wouldn't be possible to install software as unprivileged user.

It's up the application to decide how to use prefix variable. Most applications are respecting it. But you make it sound like it's impossible to not respect it, which is not true. I can write automake rules which completely ignore prefix.
