Currently, an entire project exists to fill a gap that OSM could and
should be able to provide very easily:

Right now it is possible to search for nearby toilets, but if you
specifically want to search for wheelchair-accessible and/or all-gender
bathrooms near a location, there is no (clear) way to do so.
