I ran into a similar issue recently while using a US geofabrik extract; addresses in Philadelphia would return as being in New Jersey, and some other state boundaries were also not reporting correctly when geocoding. I imported the states and re-indexed, which didn't work for me. I also tried making the extract myself from the full planet file, with the same result. Eventually I wound up just using the full planet file.
