The configuration looks good at a first glance.
Maybe there's a problem in the ImageMosaic or Pyramid itself?

Have you the chance to rise up the GeoServer logging level to VERBOSE and
try to reproduce the issue? Than if you can provide us the StackTrace it
may be useful to better spot the issue.
