Agreed, it does seem to be missing. But I altered the URL slightly to point
to the default style "point" instead and it works ok.

So I guess the issue is in the sld file, not the lack of "&layer=".
Odd - the SLD spec says that "LAYER" is "Required", so GeoServer should bug
out when its not there. I'll open a bug report for that one, though that's
not necessarily the problem here.
