Hi Andy, looks like you're missing some EHCache jars, Not sure why though, the GeoServer war should have it built-in?
Regardless, the module in question has been abandoned for some time, the original authors left and no one took it over. In the meantime other members of the community worked on a new set of COG support modules, that is landing in the 2.18.2 release (to be released this week, maybe today).
