That was the problem; however I was trying to do an inline image as shown here
(http://docutils.sourceforge.net/docs/ref/rst/restructuredtext.html#substitution-definitions):
The |biohazard| symbol must be used on containers used to dispose of medical
waste. .. |biohazard| image:: biohazard.png
