Ciao Jeff, I guess I would need to look at the code you are writing in order to be helpful.
Anyway, what you need to do is to make use of the Read_gridgeometry read parameter of the MRsidReader with the envelope and pixel dimensions needed. See code below, where you have code to set many parameters to control the reading process: http://pastebin.com/EYmqMgGZ if you are using the geotools renderer, it should handle that seamelessly.
