For the knowledgebase: It turns out there is a simple solution: what I didn't know is that Plone does html filtering, and that <form> and <input> are on the default list. I just went to the Plone site settings, deleted these two from the html filtering list and viola! the form works. Thanks to ajung on the Plone user list for this help.
