I stopped and looked at the source code, and it had ghw.c, ghw.h and ghwlib.h in it. I figured I had a problem with how I was testing it, so I dumped all the libraries in the same directory with the binaries invoked it from there and it worked. (It works if you install the disk image too, I was trying to avoid that). There are only 5 libraries there as opposed to the 44 I found in the latest Macports gtkwave release. So there is a gtkwave viewer for Mac OS X accessible from eng-osx. Signals are selected for viewing through Search => Signal Hierarchy display formats are under Edit => Data Format. You can select X11 fonts and other parameters using a .gtkwaverc file, one of which is in the distribution source file. For other controls not present in it, see the gtkwaverc.5 man page.
