Stumbled across this earlier today. 'Free Range VHDL' Bryan Mealy and Fabrizio Tappero PDF 151 pages, 2.6 MB, licensed under the Creative Commons Attribution-ShareAlike Unported License. The LaTex source is available. Described on the Front cover as "The no-frills guide to writing powerful code for your digital implementations". The PDF shows a creation date of November 29, 2011 and the web site showed up in the last week.

I'm reminded of 'VHDL: Hardware Description and Design' Roger Lipsett, Carl Schaefer and Cary Ussery, which was quite popular. (Oh, and ghdl is mentioned on page 4).
