Stumbled across this earlier today.

'Free Range VHDL' Bryan Mealy and Fabrizio Tappero PDF 151 pages, 2.6 MB,
licensed under the Creative Commons Attribution-ShareAlike Unported License.
The LaTex source is available. Described on the Front cover as "The no-frills
guide to writing powerful code for your digital implementations". The PDF
shows a creation date of November 29, 2011 and the web site showed up in the
last week.

You could nitpick like crazy about unsubstantiated or loose facts, but that's
not the point. It's from a practical and introductory point of view.

I'm reminded of 'VHDL: Hardware Description and Design' Roger Lipsett, Carl
Schaefer and Cary Ussery, which was quite popular.

(Oh, and ghdl is mentioned on page 4).
