Custom filetypes are not very powerful, they can't do anything that the editor (Scintilla, mostly for highlighting) or the tag parsers (symbol list) can't already do. However Scintilla's parsers (lexers) are configurable to an extent - to really know all the settings you need to read the lexer source code, but that may be overkill. So if you think this approach may be fruitful, you need to find an existing filetype and look at what settings are there.
