   - make direct remote channel use manual flow control with a larger flow 
window
   - remove unused method
   
   in testing, internal gRPC buffering can stall streaming gRPC calls by 
O(minutes) on the commit path leading to O(minutes) of latency in system and 
data watermark.  Increase the flow control window to match the dispatched 
remote channel.
