I figured part of it out.
I ran 
   
sudo gitlab-rake gitlab:check

and found out the hooks subdirectory was misconfigured for one project. I 
fixed this using 

/opt/gitlab/embedded/service/gitlab-shell/bin/create-hooks

and now pushing to this project triggers an email.

New users still don't get a confirmation email though, so any help on that 
would still be appreciated.
