Does your regular Rx have an ADD power for reading? If so, you'd split that value by half and adjust the SPHERE in your Rx accordingly. You'd leave the CYL and AXIS as is in your Rx.
For example: Let's say your ADD power was +3.00 You would split it in half, so it would be +1.50 You'd take the +1.50 and ADD it to the SPHERE of your Rx.
