Can't speak for Cairo, but (GTK or whatever)+DirectFB+AGG would theoretically be possible and was discussed a few times. However, nobody actually coded it yet (should be rather simple for somebody familiar with DirectFB). Note that you can also run Gnash in fullscreen without GTK, DirectFB, X11 since it can access the frame buffer directly.
