you ask for the make and model of the video adapter card. I
have only heard Jarod specify the make ("Nvidia is good, ATI sucks"), so
having the model number is more information than I can check.
I was thinking it might still be useful for you to have that
information in your "files". If we end up needing it, someone else
(with the know) can check.

The latter. Upon a successful form submission, the submitted data
is mailed to [EMAIL PROTECTED] only.
In any case I think it would be good to email the person
registering a copy of the form also ...
Hmmm. The submission script I'm using (TFmail) doesn't appear to be
able to do that. I'd have to modify the code. Looking at said code,
I think I'd have to do some major restructuring, as it starts building
the email stuff way early on, before it processes the form submission
(where the user's email address is). Are we willing to wait?
