Because you'd selected Subtotal on both options as you described. This is a long-standing wart/bug because it's nonsensical to allow subtotals to be shown both before and after each account+subaccounts. And it's impossible to fix this behaviour without angering many long term users. The "next-gen" balance sheet is in the experimental reports menu; the latter will never show subtotals twice.
