Duplicate transactions could happen if the user isn't carefully reviewing what the matcher has cooked up during importing of account data from the FI to GnuCash. EG if I import credit card data, payments to my credit card are automatically attributed to my checking account. Then, If I import data from my checking account data, a failure in matching promotes the same transaction to be added again because the existing transaction isn't matched to the transaction being imported. The matcher's automation is good for the majority of the work, but the user still needs to review its work and occasionally correct things. Anyway, perhaps a scenario like this is causing your duplicates?
