I have gotten my self into this situation at times and the culprit that I find is that a transaction amount provided is off by large value even though quantity is correct, for example VFIAX being purchased at $1.00 rather than $377.71 (today's price). This will throw off the subtotal by quite a large amount and this mistake happens unintentionally as a wrong key by mistake is depressed.

I did try to get the transactions with a CVS and an QFX files but no transactions were found. I went through all of the Mutual Funds in the pop ups. There were no results. I checked the entries several times to make sure that the price data were correct and the fund shares quantities matched. I also entered the data in a spread sheet which confirmed the statements.
