It seems you have inadvertently expanded the hidden Rate column beyond its usual 1 pixel width - see this thread for details - https://lists.gnucash.org/pipermail/gnucash-user/2021-January/095003.html
Judging by the thick line to the left of the "R" (Reconcile) column you perhaps have reduced that to a 1 pixel width at some tome. Cheers, the other David.
