Ahh bingo, I'll give that a try in a moment.  I was looking at the `pinentry` documentation, which was the completely wrong place to look for this setting.

A follow up, this was indeed the fix. Debian seems to ship a `gpg-agent` binary that defaults to `grab`. Adding this one line and rebooting (overkill, but I had a pending kernel update) fixed the issue.
