You need to specify the icon as base 64.

This tool can help.

The problem I had before was that I used a png for the tool, and the icon
worked for firefox but not IE.  So I then went and used an .ico or icon file
format and it worked.

Simply upload the file to this site and paste the base 64 in the contents.

We have an accessibility tool that analyzes content so people with
disabilities can get access.  This online version we are letting developers
use one page at a time for free, and I wanted a way that would be easy for
