It's not your fault. The problem is Picasa doesn't handle synthetic
keystrokes when the keystrokes involve modifiers like ctrl and alt.

Assuming this is on your media computer, where Picasa would always be
maximized - many of the controls are always at the same locations. You
can figure out the coordinates, and just click them instead. i.e.
in .lircrc, put

config = Button 1 x y CurrentWindow

insteadl of Key ctrl-4 CurrentWindow

where x and y are the positions for the play slideshow button.
