I know I'm wading in quite late in the day here, but where are you
running your php script?

I have a php page with static maps in it; if I run it through
localhost on my development machine, the page comes up with an empty
image. Deploy the page to my live web hosting environment, and the
image is there (with *exactly* the same external Google URL).

I remember reading other posts on this and it was a localhost / API
key issue, which I wasn't able to resolve.
