It's not all that difficult. I've had this primitive "proof of concept" demo online for ages: http://maps.forum.nu/gm_driving_directions2.html and this one has also been around for a long time: http://www.marsrutai.info/directions.htm The best version I know of is one I've implemented on a clients website. It has the exact same functionality as maps.google.com, including drag-and-drop on the sidebar to change the order of the destinations.
