The minimum required CCR for adding a medication to a profile is the
following:

<ContinuityOfCareRecord xmlns='urn:astm-org:CCR'>
  <Body>
    <Medications>
      <Medication>
        <Product>
          <ProductName>
            <Text>new_medication</Text>
          </ProductName>
        </Product>
      </Medication>
    </Medications>
  </Body>
</ContinuityOfCareRecord>

If you post this CCR to the register feed, you should see
"new_medication" in your profile.  To maximize the portability of the
health information that you're uploading, you'd likely want to add an
RxNorm or NDC code; although this isn't required.  If you supply a
code that Health recognizes, you can omit the medication name and
Health will fill it in.
