As far as my experience says, RBL + ORBS effectively block almost all
spam mails. However, the error messages that sendmail produces when
rejecting e-mail due to ORBS or RBL query are pretty brief and - this
was the case mainly with ORBS which includes also "suspicious hosts"
that even bigger providers and companies sometimes run - most "lusers"
complain that your mail host is misconfigured and rejects all mail
from their site. I was already thinking about tweaking ORBS ruleset in
sendmail to produce a more verbatim message, but currently I've enough
other things to do.

I'd go for RBL in any case, and I can only recommend ORBS (but
filtering with ORBS will occasionally give you some additional work of
explaing why things are as they are).
