You asked about an IRB amendment regarding insurance and such for the obesity 
survey. Alex answered that it's covered by the GPC DSA. I just double-checked, 
and I concur.

The GPC 
DSA<https://informatics.gpcnetwork.org/trac/Project/wiki/GovernanceMaterials#DSA>
 involves an approved DROC request. The relevant one seems to be #6 from Dan 
Hale 08/18/2015. That request makes no explicit mention of insurance, though it 
does refer to "global IRB protocol for the obesity study".

In Table 7.  Outcomes/Data and Possible Correlations of that protocol, under  
Data gathered from the EHR at each site, I find: Type of insurance.
